How does urinary PdG relate to serum progesterone, and is it normal for PdG to fluctuate?Updated 3 months ago
Urinary PdG (Pregnanediol Glucuronide) is the primary metabolite of serum progesterone and provides a delayed reflection of progesterone activity. Because serum progesterone itself can fluctuate dramatically—studies show up to an 8-fold change within 90 minutes in the same individual—it is normal for urinary PdG to rise and fall as well.
With Mira, PdG is most useful when viewed as a trend over time rather than as isolated values. Fluctuations (dips or spikes) are common and may reflect normal corpus luteum function, stress, PCOS, or early pregnancy. These variations are generally expected and not always clinically significant.
