The affiliate form includes standard questions required by the platform, which unfortunately can’t be customized for our providers.

Please answer as best you can. It’s okay to choose the option that seems closest to your situation, even if it’s not a perfect fit. The information you enter here does not affect your partnership with Mira; it’s simply a formality for the affiliate platform.

Question: How would you categorize your business?

If any of the categories clearly describe how you reach patients or followers, choose that option. If none of them fit, or you’re unsure, you can select “Email/Newsletter.” This is an easy default option that works well for most Mira providers. 

Question: Which methods do you use to promote? (Choose all that apply.)

Select whichever channels you currently use or are comfortable listing. If you’re not sure, you can check “Email newsletter” or optionally “Website” or “Social Media” if you have those. It’s fine to choose a combination that best reflects how you might share Mira information — even if you don’t actively use those channels at the moment.
